E-Disclosure Program (Electronic Delivery of Required Notices)
Good news if you’re tired of extra paper: the New York State Teamsters Benefit Funds now offer a no-cost option to receive certain required plan notices electronically instead of by mail. These can include Summary Plan Descriptions, Summaries of Material Modifications, Summary Annual Reports, and Annual Funding Notices.
- How it works: After you enroll, we’ll email you when documents are posted online (and you’ll get an initial email showing what’s already available).
- Access: You can view documents online and download, save, or print them anytime.
- Change your mind? You can withdraw consent at any time and go back to paper mail at no charge.
How to enroll: Fill out the Consent Form and send it by mail to New York State Teamsters Benefit Funds, PO Box 4928, Syracuse, NY 13221, or email the completed form to [email protected].
