Form 1095-B

Form 1095-B is used to report certain information to the IRS and to taxpayers about individuals who are covered by minimum essential coverage.  Taxpayers are not required to include Form 1095-B when filing their taxes and should not be attached to a tax return.

Updates to Form 1095-B

The Internal Revenue Service (IRS) no longer requires taxpayers to provide Form 1095-B with their tax return and no longer requires health insurers to mail them to members. You can still request a copy for your own records by contacting the Fund office at (315) 455-9790 or by mailing a request to:
